As your skin ages, your skin loses collagen and fat and falls prey to other factors such as sun damage, smoking, poor diet and pollution. All of these things can take their toll on your anti-ageing wellness, causing your skin to appear wrinkly and saggy. However, dermal fillers are non-surgical injections that claim to restore youthfulness to your face, but how do they work? We spoke to celebrity cosmetic dermatologist Dr Shuba Dharmana, the Founder of Le’Jeune Medspa and winner of Woman Entrepreneur of the year 2012, to find out more.


 


According to Dr Dharmana, ‘Dermal fillers contain hyaluronic acid in a very purified form. This is used to build collagen that is lost from the ageing skin thus restoring volume, lifting sagging skin and plumping out the folds. The hyaluronic acid filler is injected in to the layers of skin, in the folds after numbing the skin with a local anaesthetic cream…These injections are not very painful when used with the numbing cream and you may feel only a slight discomfort. But the effects of the fillers last for 12-18 months and they help maintain younger looking skin.’ So which areas can be treated with dermal fillers?


 


1. Cheeks: ‘As we age, we lose volume in the cheeks,’ says Dr Dharmana. ‘This, in addition to the effects of gravity causes the underlying tissues and the skin to droop giving rise to laughter folds. This volume in the cheeks can be regained with thicker dermal fillers that not only create a fuller, youthful lift giving high cheek bones but also correct the laughter lines.’


 


2. Lips: Dr Dharmana explains, ‘The injections used in the lips will help accentuate the lip line, the cupid’s bow area, the smoker’s lines and to plump body of the lips. Depending on one’s pain threshold, they may choose to have this with a local anaesthetic cream or have a set of numbing injections called ‘dental blocks’ given into the gums inside the mouth prior to the procedure. These injections given inside the mouth are relatively painless and are similar to an injection from your dentist before a tooth removal.’


 


3. Décolletage: ‘We often overlook our décolletage, the most age revealing part of our bodies,’ Dr Dharmana laments. ‘It is the area comprising the neck, shoulders, cleavage. The skin around the cleavage is one of the first places women show their age. The skin is fine, thin and very delicate, and tends to show sunspots and lines easily with age. Women also tend to overlook this area when it comes to skincare, not realising they should be treating it with just as much care as their face. This area can be treated effectively with a slightly less viscous dermal filler like Juvederm Hydrate, Restylane Vital, Mesolis or IAL. You will have a series of tiny injections in the area after using a numbing cream and you will require three sessions and maintenance treatment once every three months.’


 


4. Nose: Dr Dharmana asks, ‘Don’t like the shape of your nose? You can correct it without surgery with this treatment. Fillers are used to non-surgically repair a crooked nose or build a higher nasal bridge. This non-surgical rhinoplasty performed with fillers lasts up to two years.’


 


5. Hands: ‘Hands, especially fingers show signs of ageing which can be corrected with the help of a filler that hydrates, lifts and plumps volume in to the sagging skin,’ notes Dr Dharmana.


 


But what about possible side-effects? Dr Dharmana adds, ‘Due to the use of needle, there is likely to be some bleeding and rarely some bruising at the injection site. Redness and swelling may occur after the injection and this reaction may last for two weeks. There is rarely any discolouration of the injection site, nodules. But if these develop, report them to the practitioner as soon as possible so they can advise on the best course of treatment.’

1. Bacne: According to cosmetic dermatologist Paul Frank, MD, ‘Back acne often has the same triggers as facial acne – hormones spur an increase in oil production, and dirt and sweat clog pores – yet it’s typically tougher to treat because creams are rubbed away when you put on clothes or lie down. And this type of acne can be extreme because the skin on that part of the back is thicker and the hair follicles are larger, creating a perfect breeding ground for bacteria.’ So how do you fix it? Beauty and wellness expert Nicole Catanese advises, ‘Grab a loofah that has a handle or attach the Clarisonic Body Brush Extension Handle ($25, clarisonic.com) to your go-to face tool, then lather up your back with an acne wash that contains salicylic acid or benzoyl peroxide to unclog pores.’


 


2. Chicken Arms: ‘You do a gazillion tricep dips for sexy arms, yet you’ve got stubborn red bumps,’ notes Catanese. ‘Why do you have “chicken skin” on your arms but your friend doesn’t? Genetic roulette. But you’re not alone: About half of women have what are called keratosis pilaris, or KP.’ To beat the little red bumps, dermatologist Jeannette Graf, MD, recommends daily buffing with gentle exfoliators that have fine granules and hydrators like glycerin and hyaluronic acid. She explains, ‘These products break down dead skin on the surface, clearing the hair follicles.’


 


3. Stretch-Mark Legs:  Dr. Graf elaborates, ‘When skin grows faster than it was meant to – from rapid weight gain – its network of collagen fibres becomes uneven and creates visible indentations.’ To rid your legs of stretch marks, Catanese advises, ‘Use the same retinol (or Rx retinoid) or peptides that you use to firm your face, or try skin smoothers with silicone, such as dimethicone. To prevent stretch marks during pregnancy, slather on creams with moisturizing cocoa butter and hyaluronic acid, then follow up with a body oil.’


 


4. Spider Veins: M. Dean Vistnes, MD, the founder of SkinSpirit skin-care clinic and spa and a plastic surgeon in Palo Alto, California, points out, ‘The tiny spider veins on your calves and the backs of your thighs tend to get worse with age because the small blood vessels there don’t work as well as they once did and become dilated.’ Medically speaking, you have nothing to worry about. However, if you don’t like the sight of your spider veins, Catanese suggests, ‘Hide veins temporarily with body makeup like St. Tropez Self Tan Perfect Legs Spray ($18, sttropeztan.com). 5. Cellulite: Catanese laments, ‘No matter how good you are about eating salad for lunch and saying no to a second piece of cake, the cellulite on the back of your thighs and rear stays put. Cellulite is actually a skin condition, which explains why even women who are a size two can have it.’ Dr. Frank adds, ‘Foods that are acidic or cause inflammation, such as diet soda and processed snacks like chips, cause fat cells to swell, making cellulite look worse.’ So cut down on your intake of inflammatory foods. Catanese also recommends, ‘Regular leg workouts like squats and lunges help rev up circulation, which also keeps puffiness at bay.’

Cosmetic surgery can seem like a big step to take in the name of anti-ageing wellness. I mean, we all want to look young and gorgeous, but are we really willing to put our wellbeing at risk and go under the knife to do it? According to the American Academy of Facial Plastic and Reconstructive Surgery, about 80% of board-certified facial plastic surgeons have now reported an increase in non-invasive cosmetic procedures, meaning that you can lift your cheekbones, eliminate under-eye bags and dark circles, and replicate the results of a nose job, without facing the dreaded scalpel.


 


The new non-invasive procedures all have one thing in common; replacing volume. Fredric Brandt, a New York City and Coral Gables, Florida, dermatologist and founder of Dr. Brandt Skincare, asserts, ‘Nothing makes people look more youthful than filling in volume, and as we get more and more experience with the newer injectables, it seems like we are able to achieve better results without surgery.’ Patricia Wexler, a New York City dermatologist, adds, ‘What I see most in my practice is a large amount of volume loss in the face, which tends to happen in your early 40s, and that is when you replace a surgical procedure with a non-surgical one.’


 


Fardad Forouzanpour, founder of the Beverly Hills Cosmetic Surgical Group, points out, ‘For the past few years, the trend has been more natural looking lips, [and] injectables can give lips more volume and correct lines and wrinkles that form around the mouth.’ For the most natural-looking results, Brandt believes Restylane and Perlane hyaluronic acid-based fillers to be your best bet as they provide a cumulative effect. Wexler agrees, ‘I love Restylane and Perlane for the tear troughs, cheeks, and jawline because of the volume and lift they give to the face.’


 


However, Forouzanpour notes that the more important thing to consider is your physician’s injection technique and the amount that is injected, rather than what the kind of filler is. Wexler comments, ‘If you have cracks at the corner of your mouth or above the lip line, collagen works well to replace structure in the skin. People don’t think about collagen anymore because they are so into hyaluronic acid. Although I use Restylane to add volume, I need collagen to create a border around the lip [cupid"s bow].’


 


So which fillers are the best, then? Wexler explains, ‘Doctors pick certain ones and get proficient at using them. In 2003, we had collagen — period. To do the right kind of filling now, one syringe of anything won’t be complete.’ As it stands, the AAFPRS states that there are almost 20 FDA-approved cosmetic injectable drugs on the market, so how do doctors choose their favourite filler? ‘Besides filling in volume, we need to think about surface irregularities,’ Wexler says. ‘Even the most plump face won’t be pretty if the skin isn’t in good condition.’


 


So when considering getting fillers, you first need to make sure your skin is right for it. Wexler argues, ‘If a 68-year-old woman who has never taken care of her skin and spent a lot of time in the sun comes to me, injectables won’t give her enough of an improvement if she has loose skin and sun damage. There’s a point when you have to start from a new baseline, and if the person is healthy, she would need a face-lift.’ If your skin is filler-ready, however, you need to do your homework before getting any procedure. This means checking out your doctor’s background and reputation before you get injected. Wexler warns, ‘A lot of my work is fixing other people’s work.’

While no one has even found the Fountain of Youth, many have tried to bottle it and sell it.  According to Board Certified Plastic Surgeon Hilton Becker, MD, ‘The problem with many products is multi-fold. Many are not created by physicians with the patient in mind, but instead created in labs by people who are not at all familiar with combating the effects of ageing.’ Therefore, Dr Becker has developed and marketed his own brand of anti-ageing products, which he believes to be the finest skin care products available for his patients.


Dr Becker believes that many chemicals often found in anti-ageing products – such as fragrances, colouring agents, thickeners and emulsifiers – do nothing for your wellbeing, and are only added in to make the product more marketable.
